The file involved included about 18,000 names of current and former Ohio State University students who were enrolled in the university-sponsored Student Health Insurance Plan during the 2005-2006 academic year (Autumn 2005 through Summer 2006). On Dec. 30, 2008, the University mailed (via U.S Postal Service) letters to all students who were enrolled in the Student Health Insurance Plan during any of those four quarters. The addresses used were the “home” addresses supplied by the students to the University Registrar, with the exception of international students, for whom “local” addresses were used. The information was erroneously posted on an Internet server maintained by a company that had been doing work on behalf of Ohio State. The files and caches have been removed, and there is no evidence of fraudulent activity.
